What is COPD Rescue Pack?
The COPD Rescue Pack is a combination of standby medications prescribed to patients with Chronic Obstructive Pulmonary Disease (COPD) for emergency use during acute exacerbations. It contains two essential components:
- Prednisolone 5mg tablets - A corticosteroid that reduces airway inflammation
- Either Amoxicillin 500mg capsules - A penicillin-type antibiotic for bacterial infections
- OR Doxycycline 100mg capsules - A tetracycline antibiotic alternative for penicillin-allergic patients
Mechanism of Action
Think of your airways as inflamed, narrow tubes during a COPD flare-up. Prednisolone works like a powerful anti-inflammatory agent, reducing swelling and inflammation in the airways, much like how ice reduces swelling in a sprained ankle. Amoxicillin or Doxycycline act as infection fighters, targeting and eliminating bacterial infections that often trigger or worsen COPD exacerbations.
Biological Pathway
During COPD exacerbations, the body's inflammatory response goes into overdrive. Prednisolone enters cells and binds to glucocorticoid receptors, inhibiting the production of inflammatory proteins called cytokines. This reduces inflammation, mucus production, and airway swelling. Meanwhile, antibiotics interfere with bacterial cell wall synthesis (Amoxicillin) or protein production (Doxycycline), eliminating bacterial pathogens that can worsen airway inflammation.
What is COPD Rescue Pack Used For?
The COPD Rescue Pack is specifically designed for managing acute exacerbations of Chronic Obstructive Pulmonary Disease when immediate medical attention is not available.
Approved Indications
| Symptom Type | Medication to Start | Clinical Criteria |
|---|---|---|
| Non-infected worsening | Prednisolone ONLY | Increased breathlessness, increased cough, increased sputum WITHOUT colour change |
| Infected exacerbation | Prednisolone + Antibiotic | Above symptoms PLUS coloured sputum, fever, shivering |
Target Patient Populations
- Patients with diagnosed COPD (GOLD stages 2-4)
- Those with a history of frequent exacerbations (≥2 per year)
- Patients living remotely from immediate medical care
- Individuals with an established COPD management plan
CRITICAL SAFETY WARNING
This pack is for EMERGENCY use only when you cannot immediately see your GP. You MUST contact your healthcare provider within 24 hours of starting the pack. If symptoms worsen or emergency symptoms develop (severe breathlessness, chest pain, confusion), seek immediate medical attention by calling 999.
Before Using COPD Rescue Pack
Before being prescribed a COPD Rescue Pack, you should have:
Medical History Assessment Checklist
- ✓ Confirmed COPD diagnosis by a healthcare professional
- ✓ Established COPD management plan in place
- ✓ Known allergy status documented (especially penicillin/antibiotics)
- ✓ Regular review of COPD medications and inhaler technique
- ✓ Understanding of when to use the rescue pack
Required Baseline Considerations
- Allergy Status: Must be documented (penicillin, amoxicillin, tetracycline allergies)
- Current Medications: Review for potential interactions
- Comorbidities: Diabetes, osteoporosis, hypertension, glaucoma, peptic ulcers
- Infection History: Previous antibiotic use and resistance patterns
COPD Rescue Pack Contraindications
Absolute Contraindications (DO NOT USE)
- Amoxicillin-containing pack: Known hypersensitivity to penicillin, amoxicillin, or other beta-lactam antibiotics
- Doxycycline-containing pack: Known hypersensitivity to tetracycline antibiotics
- Previous severe reaction to corticosteroids (anaphylaxis, severe skin reactions)
- Systemic fungal infections (unless being treated)
- Live virus vaccinations while on corticosteroids
Relative Contraindications (Use with Extreme Caution)
| Condition | Risk Level | Precautions Required |
|---|---|---|
| Diabetes mellitus | High | Monitor blood glucose closely; may need insulin adjustment |
| Osteoporosis | Medium-High | Short-term use only; consider calcium/vitamin D |
| Peptic ulcer disease | Medium | Take with food; consider PPI cover |
| Hypertension | Medium | Monitor blood pressure; may cause fluid retention |
| Glaucoma | Medium | Monitor eye pressure |
Special Population Considerations
Elderly Patients (≥65 years)
No dosage adjustment typically needed for short courses. However, monitor closely for:
- Increased fall risk (steroid-induced myopathy)
- Confusion or mood changes
- Fluid retention and hypertension
- Glucose intolerance
Renal Impairment
- Amoxicillin: Reduce dose in severe renal impairment (eGFR <30)
- Doxycycline: No adjustment needed
- Prednisolone: No adjustment needed
Hepatic Impairment
- Use with caution in severe liver disease
- Prednisolone may exacerbate hepatic encephalopathy
- Monitor liver function if pre-existing impairment
Pediatric/Adolescent Patients
NOT RECOMMENDED. COPD is primarily an adult disease. This rescue pack is not indicated for pediatric use. Doxycycline is contraindicated in children under 12 years due to teeth discoloration risk.
COPD Rescue Pack and Other Medicines
High-Risk Medication Combinations
| Medication Class | Interaction with Rescue Pack | Recommendation |
|---|---|---|
| Warfarin/Anticoagulants | Amoxicillin may increase INR; steroids may alter response | Monitor INR closely; may need dose adjustment |
| Methotrexate | Amoxicillin may increase methotrexate toxicity | Avoid combination if possible; monitor blood counts |
| Oral Contraceptives | Antibiotics may reduce contraceptive efficacy | Use additional barrier method during and 7 days after antibiotics |
| Diuretics | Steroids cause potassium loss; additive effect with diuretics | Monitor potassium levels; may need supplementation |
| NSAIDs | Increased risk of GI bleeding with steroids | Use with caution; take with food; consider PPI cover |
| Antacids/Iron Supplements | Reduce absorption of Doxycycline | Take Doxycycline 2 hours before or 4 hours after these products |
Timing Recommendations
- Doxycycline: Take 2 hours apart from antacids, calcium, iron, or dairy products
- Prednisolone: Take with food in the morning to mimic natural cortisol rhythm
- Amoxicillin: Space evenly throughout the day (e.g., 8-hour intervals)
Pregnancy & Breastfeeding
Pregnancy Warning
PREDNISOLONE: Can be used in pregnancy if benefits outweigh risks, but high doses or prolonged use may cause fetal issues.
AMOXICILLIN: Generally considered safe in pregnancy.
DOXYCYCLINE: CONTRAINDICATED in pregnancy - causes permanent teeth discoloration and inhibits bone growth in fetus.
Always inform your doctor if you are pregnant, planning pregnancy, or breastfeeding before using any medication.
Breastfeeding Considerations
- Prednisolone: Small amounts enter breast milk; considered compatible with breastfeeding
- Amoxicillin: Compatible with breastfeeding; may cause infant diarrhea or thrush
- Doxycycline: Generally avoided; theoretical risk of teeth discoloration
Contraception Requirements
Antibiotics may reduce the effectiveness of hormonal contraceptives. Use additional barrier contraception (condoms) during antibiotic treatment and for 7 days after completion.
Driving & Machine Operation
Prednisolone may cause:
- Dizziness, vertigo
- Visual disturbances
- Psychiatric reactions (euphoria, insomnia, mood swings)
Warning Signs to Watch For:
- Feeling dizzy or lightheaded
- Blurred vision
- Difficulty concentrating
- Unusual mood changes
RECOMMENDATION: Do not drive or operate machinery until you know how these medications affect you. If you experience any of the above symptoms, avoid potentially dangerous activities.

COPD Rescue Pack FAQs
When exactly should I start my COPD Rescue Pack?
Start your rescue pack when you experience increased shortness of breath, increased cough, and increased sputum that prevents normal activities. If sputum changes colour or you develop fever/shivering, start both steroid and antibiotic. Always follow your specific COPD action plan and contact your GP within 24 hours.
Can I take the rescue pack if I'm allergic to penicillin?
NO. If you are allergic to penicillin, you must NOT take the Amoxicillin-containing pack. You should have been prescribed the Doxycycline-based pack instead. Always check which antibiotic is in your pack before taking. Inform your doctor of any antibiotic allergies before receiving a rescue pack.
How quickly should the rescue pack work?
You should notice improvement within 2-3 days of starting treatment. Prednisolone reduces inflammation quickly (within 24 hours). Antibiotics may take 48 hours to show effect. If no improvement within 3 days or symptoms worsen, contact your GP urgently or seek emergency care.
Do I need to taper the Prednisolone after a 7-day course?
No. For short courses of 7 days or less, you can stop Prednisolone abruptly without tapering. Tapering is only necessary for courses longer than 3 weeks, as your body needs time to restart natural cortisol production. Always follow your doctor's specific instructions.
Can I drink alcohol while taking the rescue pack?
It's best to avoid alcohol during treatment. Alcohol can increase stomach irritation with Prednisolone, may reduce antibiotic effectiveness, and can worsen COPD symptoms. If you do drink, limit to small amounts and take with food. Alcohol can also impair judgment about symptom severity.
What should I do if I develop diarrhea from the antibiotics?
Mild diarrhea is common. Stay hydrated, eat plain foods (bananas, rice, toast), and consider probiotics. STOP antibiotics and contact your GP immediately if you develop severe/watery diarrhea, abdominal pain/cramps, or fever, as this may indicate pseudomembranous colitis, a serious condition.
Can I get a repeat rescue pack without seeing my doctor?
After using a rescue pack, you must inform your GP to get a replacement. Some GPs may authorize repeat prescriptions, but you'll need regular reviews. Through our online service, you can request a replacement after consultation with our clinicians, who will review your medical history and need.
How should I take Doxycycline to avoid stomach problems?
Take Doxycycline with a full glass of water (200ml+) while standing or sitting upright. Remain upright for 30 minutes after. Take with food if nausea occurs, but avoid dairy, antacids, or iron supplements within 2 hours. May cause photosensitivity - use sunscreen.
What if I start the pack but then feel worse?
If symptoms worsen or you develop severe breathlessness, chest pain, confusion, drowsiness, or high fever, seek immediate medical attention. Call 999 or go to A&E. Do not wait for the medications to "kick in" if you're getting worse. This could indicate a serious complication.
Can I take other medications with my rescue pack?
Continue your regular COPD medications (inhalers, etc.). However, some medications interact - particularly warfarin, methotrexate, and certain diabetes medications. Always inform your healthcare provider about all medications you're taking before starting the rescue pack to avoid dangerous interactions.
How long does a rescue pack last before expiring?
Typically 1-2 years from manufacture, but always check the expiry date on your specific pack. Do not use if expired. Store properly to maintain potency. Request replacement before expiry if unused. Expired medications may be ineffective or harmful.
Should I increase my inhalers when using the rescue pack?
Yes. Increase your salbutamol (blue reliever) inhaler use as needed during exacerbations - typically every 4 hours or as directed in your action plan. Continue your preventer inhalers (brown/orange) as normal unless advised otherwise by your healthcare provider.
Can I get a rescue pack if I have asthma instead of COPD?
Rescue packs are primarily for COPD. Asthma exacerbations are managed differently, often with higher dose inhaled steroids or oral steroids alone. Consult your doctor for an asthma-specific action plan. Do not use a COPD rescue pack for asthma without medical advice as treatment differs.
What monitoring is needed while taking the rescue pack?
Monitor symptom improvement, watch for side effects, and check temperature if fever was present. Diabetics should monitor blood glucose closely. Contact your GP within 24 hours of starting pack and if any concerns arise. Keep a symptom diary to track progress.
Where should I keep my rescue pack?
Keep in a cool, dry place easily accessible during flare-ups. Many patients keep it with their COPD action plan. Ensure family members know its location in case of emergency. Take it with you when traveling away from home. Keep out of reach of children.
