Your Obligations
- Thoroughly read and understand our Patient Guide available at www.chemistdoctor.com before consenting to any treatment.
- Provide complete, accurate and timely information relevant to your treatment requirements.
- Complete all registration forms and medical questionnaires truthfully and comprehensively.
- You must be aged 18+ to use our services. Providing false information will immediately invalidate this agreement.
Treatment Understanding
- Consultations are specific to the requested health service and not general health screenings.
- By requesting treatment, you confirm informed consent and acknowledge receiving sufficient information about its appropriateness.
- No medical test is 100% accurate and treatment outcomes cannot be guaranteed.
- Initial treatment courses may require follow-up treatments at additional cost if ineffective.
Medication Management
- Use prescribed treatments exclusively for personal use
- Contact our practitioners through your Patient Record for:
- Clarification about medication instructions
- Reporting side effects or treatment concerns
- Do not commence treatment until receiving clarification from a practitioner when uncertain.
Account Security & GP Communication
- Maintain account security – never share login credentials
- Create only one account per individual
- We strongly recommend informing your NHS GP about treatments received
- We can notify your GP upon your request
Patient Declaration
I confirm that:
- I’ve recently consulted a medical professional
- I understand the risks, benefits and potential side effects of requested medications
- All provided information is truthful and complete to the best of my knowledge
- I will inform my GP about any prescribed medications
- I understand that providing inaccurate information could lead to harmful medical advice
