What is Mercilon?

Mercilon is a combined oral contraceptive (COC) pill belonging to the pharmacological class of hormonal contraceptives. Each white tablet contains two active synthetic hormones:

  • Desogestrel (150 micrograms): A third-generation progestogen.
  • Ethinylestradiol (20 micrograms): A synthetic oestrogen.

This combination is classified as a low-dose pill due to its 20μg oestrogen content. It is manufactured by Organon and is supplied in packs of 21 tablets.

How Does Mercilon Work? (Mechanism of Action)

Mercilon works through three primary mechanisms to prevent pregnancy:

  1. Inhibition of Ovulation: The hormones suppress the release of hormones from the pituitary gland (FSH and LH), preventing an egg from being released from the ovaries.
  2. Thickening of Cervical Mucus: The progestogen (desogestrel) makes the mucus in the cervix thicker, creating a barrier that is difficult for sperm to penetrate.
  3. Thinning of the Endometrium: The hormones make the lining of the womb (endometrium) thinner, reducing the likelihood of a fertilised egg implanting.

Analogy: Think of Mercilon as a three-layered security system for your reproductive system: it stops the release of an egg (like cancelling the release of a key participant), blocks sperm entry (like increasing security at the entrance), and makes the environment unsuitable for implantation (like removing the welcoming furniture).

What is Mercilon Used For?

Mercilon is prescribed for a single, well-defined purpose:

Primary Indication: Prevention of pregnancy.

It is suitable for women of reproductive age who are seeking a reliable, reversible method of contraception. It may also provide non-contraceptive benefits, such as:

  • Making periods more regular, lighter, and less painful.
  • Helping to reduce symptoms of premenstrual syndrome (PMS).
  • Reducing the risk of ovarian cysts, ovarian cancer, and endometrial cancer with long-term use.

Important Safety Warning

Seek urgent medical attention if you experience symptoms of a blood clot: sudden severe chest pain, breathlessness, coughing blood, swelling/pain in one leg, sudden severe headache, weakness on one side of the body, vision changes, or symptoms of a severe allergic reaction (swelling of face/tongue/throat, difficulty breathing). Stop taking Mercilon and contact a doctor immediately.

Mercilon does NOT protect against sexually transmitted infections (STIs) such as HIV, chlamydia, or gonorrhoea. The use of condoms is necessary to reduce the risk of STIs.

Before Using Mercilon

Before starting Mercilon, a thorough medical assessment is crucial. Our online clinicians will evaluate your suitability based on:

Medical History Assessment Checklist

  • Personal or family history of blood clots, heart attack, or stroke.
  • History of migraine, especially migraine with aura.
  • Current or past breast cancer or other hormone-sensitive cancers.
  • Liver, kidney, or heart disease.
  • Diabetes with complications.
  • High blood pressure.
  • High cholesterol or triglycerides.
  • History of depression.
  • Smoking status and age.
  • Current medications and herbal supplements.

Required Baseline Checks

Your doctor may recommend or you should have:

  • Blood pressure measurement.
  • Body Mass Index (BMI) calculation.
  • Up-to-date cervical smear test (as per NHS screening programme).
  • Breast examination (self-awareness is key).

Mercilon Contraindications

Absolute Contraindications (DO NOT USE):

  • Severe or multiple risk factors for arterial disease.
  • Current or history of liver disease/tumours.
  • Current or suspected hormone-dependent cancers (e.g., breast, endometrial).
  • Unexplained vaginal bleeding.

  • History of depression.
  • Inflammatory bowel disease (Crohn's, ulcerative colitis).
  • Sickle cell disease.

Special Population Considerations

Adolescents:

Mercilon can be used by adolescents once menstruation has started. Efficacy and safety are expected to be the same as in adults, but consultation with a doctor is essential.

Elderly:

Not applicable, as contraception is not intended for post-menopausal women.

Renal Impairment:

No specific studies, but no special dosage adjustments are expected. Use with caution in severe impairment.

Hepatic Impairment:

Contraindicated in active liver disease or history of liver tumours. Do not use until liver function tests return to normal.

Mercilon and Other Medicines

Several medicines can reduce the effectiveness of Mercilon or be affected by it. Always inform your doctor about all medicines you take.

Medicine TypeExamplesEffect & Recommendation
Enzyme Inducers (Reduce Efficacy) Epilepsy drugs (carbamazepine, phenytoin, topiramate, primidone, phenobarbital), Rifampicin (TB), Griseofulvin, St. John's Wort, some HIV drugs (efavirenz, nevirapine). Mercilon may not work effectively. Use a non-hormonal backup method (e.g., condoms) throughout and for 28 days after stopping the interacting medicine.
Antibiotics (Broad-spectrum) Penicillins, tetracyclines. Possible reduced efficacy. Use backup contraception during antibiotic course and for 7 days after.
Other Hormones Cyclosporin, Lamotrigine, Theophylline, Tizanidine. Mercilon can affect their blood levels. Monitoring and dose adjustment may be needed.

Pregnancy & Breastfeeding

Pregnancy

  • Contraindicated. If you become pregnant, stop taking Mercilon immediately and consult your doctor.
  • There is no evidence of harm to an unborn baby from accidental use in early pregnancy, but the pill should not be continued.

Breastfeeding

  • Not recommended.
  • Combined hormonal contraceptives can reduce milk supply and the hormones may pass into breast milk in small amounts.
  • Alternative progestogen-only contraceptives (mini-pill, implant, injection) are preferred while breastfeeding.

Contraception Requirement

Mercilon itself is a contraceptive. No additional contraception is needed if taken correctly, except when interacting medicines are used (see above).

Driving & Machine Operation

Mercilon has no known effect on the ability to drive or use machines. However, if you experience side effects such as dizziness, migraine, or visual disturbances, you should not drive or operate machinery until you feel well.

Mercilon Ingredients

Active Ingredients (per tablet):

  • Desogestrel: 150 micrograms
  • Ethinylestradiol: 20 micrograms

Other Ingredients (Excipients):

  • dl-alpha-tocopherol (antioxidant)
  • Potato starch (binder/filler)
  • Povidone (binder)
  • Stearic acid (lubricant)

Allergen Warning:

Contains lactose. If you have been told by your doctor that you have an intolerance to some sugars, contact your doctor before using Mercilon.

How to Use Mercilon

Mercilon is taken orally once a day. Adherence to the schedule is critical for effectiveness.

Standard 21/7 Regimen:

  1. Take one pill every day at roughly the same time for 21 days.
  2. Follow the arrows on the strip. Swallow the pill whole with water; do not chew.
  3. After 21 days, have a 7-day pill-free break.
  4. During this break, you will usually have a withdrawal bleed (like a period).
  5. Start your next pack on day 8, even if the bleeding hasn't stopped.

This cycle repeats continuously for as long as contraception is desired.

Mercilon Dosage Guidelines

Mercilon comes in one strength only. There is no titration schedule.

Mercilon (Desogestrel/Ethinylestradiol) FAQs

When taken correctly and consistently, Mercilon is over 99% effective. This means fewer than 1 woman in 100 will get pregnant in a year of perfect use. With typical use (occasional missed pills), the effectiveness is around 91%.

Weight gain is listed as a very common side effect (may affect more than 1 in 10 users). However, clinical studies have not consistently proven that combined pills cause significant weight gain. Any changes are usually minor and may be due to fluid retention.

Yes. To delay a period, finish your current 21-day strip and start a new strip the next day without taking the 7-day break. You may experience breakthrough bleeding or spotting. After the second strip, take the usual 7-day break.

If you vomit within 3-4 hours of taking a pill, it is considered a missed pill. Follow the instructions for being more than 12 hours late and use extra contraception for 7 days.

Fertility usually returns rapidly. You can ovulate as soon as the pill's hormones leave your system. Some women may have a short delay before their natural cycle returns, but most will be fertile within the first month after stopping.

It is safe only if purchased from a UK-registered online pharmacy that requires a consultation with a qualified prescriber, like Chemist Doctor. We ensure you are medically suitable for Mercilon before issuing a prescription, which is then dispensed by our partner pharmacy. Avoid sites that sell prescription medicines without a consultation.

There is no direct interaction between Mercilon and alcohol. However, alcohol can sometimes worsen side effects like nausea or headaches. Always drink alcohol responsibly.

No. Smoking significantly increases the risk of serious cardiovascular side effects, especially in women over 35. According to BNF guidance, women over 35 who smoke should not use combined hormonal contraceptives due to increased stroke and heart attack risk.

Breakthrough bleeding (spotting between periods) is common in the first few months. Continue taking your pills as normal. If it persists beyond 3 months or becomes heavy, consult your healthcare provider to rule out other causes.

Only enzyme-inducing antibiotics like rifampicin and rifabutin significantly reduce Mercilon's effectiveness. For most broad-spectrum antibiotics (e.g., penicillin, tetracyclines), use extra contraception during treatment and for 7 days after as a precaution.

You have a 12-hour window from your usual pill-taking time. If you're less than 12 hours late, take the pill immediately and continue normally. If more than 12 hours late, follow missed pill instructions and use backup contraception for 7 days.

Try to take your pill at the same local time each day. If traveling west, you have extra time; if traveling east, you may need to take it earlier. Use the 12-hour rule as guidance and consider taking pills with you in hand luggage.
