Complete Step-by-Step Usage Guide, Dosage Instructions & Safety Information

Key Takeaways: Using Mounjaro Correctly

  • Once‑weekly injection: Use the same day each week, any time, with or without meals.
  • Dose escalation: Start at 2.5 mg for 4 weeks, then increase to 5 mg; further increments only if needed and prescribed.
  • Injection sites: Abdomen, thigh, or upper arm – rotate each week.
  • Never share pens or needles. Always use a new needle for each injection.
  • Missed dose: If ≤4 days late, take it; if >4 days, skip and take next scheduled dose.
  • Contraindications: Do not use if allergic to tirzepatide or any ingredients, or during pregnancy.

Mounjaro (tirzepatide) is a once‑weekly injection for weight management and type 2 diabetes. Correct use ensures you get the full benefit and reduces the risk of side effects. Below we explain exactly how to use the KwikPen, who it’s for, and important safety information.

Important Medical Advice

If you experience severe abdominal pain radiating to your back, difficulty breathing, or swelling of the lips/tongue, seek immediate medical help. Always consult your doctor before starting or changing your dose.

How to Take Mounjaro: Step-by-Step Usage Guide

Follow these steps carefully each time you use your Mounjaro KwikPen. The pen contains 4 doses; you will use one dose each week.

Preparation

  1. Wash your hands with soap and water.
  2. Check the pen: the medicine should be clear, colourless to slightly yellow, with no particles. Do not use if cloudy or discoloured.
  3. Remove the pen cap.

Attach a new needle

  1. Peel off the paper tab from a new needle.
  2. Push the needle straight onto the pen and turn until it is tight.
  3. Remove the outer needle cap (keep it for disposal) and the inner cap.

Prime the pen

  1. Turn the dose knob until it stops (it will show a small symbol, often a drop).
  2. Hold the pen with the needle pointing up.
  3. Press the injection button. A drop should appear at the needle tip. If not, repeat once or twice.

Select your dose

  1. Turn the dose knob to the number of milligrams prescribed (e.g., 2.5, 5, 7.5, 10, 12.5, or 15).
  2. You will feel and hear a click as you turn.

Inject

  1. Choose an injection site: abdomen (at least 5 cm from navel), thigh, or upper arm (you may need help for the arm).
  2. Insert the needle fully into a clean area of skin.
  3. Press the injection button and hold for 5–10 seconds until the dose counter returns to 0.
  4. Withdraw the needle.

Dispose safely

  1. Carefully replace the outer needle cap.
  2. Unscrew the needle and dispose of it in a sharps bin.
  3. Replace the pen cap. Store the pen appropriately (see storage below).

🗒️ Important: Never reuse needles. Always use a new needle for each injection to avoid infection and blocked needles. Rotate injection sites each week to prevent skin lumps.

Who Should Use Mounjaro?

Mounjaro is approved for adults (18 years and older) in the following situations:

For weight management (with reduced‑calorie diet and increased physical activity)

  • BMI of 30 kg/m² or greater (obesity), OR
  • BMI of at least 27 kg/m² but less than 30 kg/m² (overweight) AND at least one weight‑related health problem, such as:
    • Prediabetes or type 2 diabetes
    • High blood pressure
    • Abnormal blood fats (dyslipidaemia)
    • Obstructive sleep apnoea
    • History of heart attack, stroke, or blood vessel disease

For type 2 diabetes

  • As monotherapy when metformin is not suitable, or
  • In combination with other diabetes medicines (including insulin) when current treatment is not enough.

Your doctor will confirm if Mounjaro is right for you based on your medical history.

Who Should NOT Use Mounjaro?

Mounjaro is contraindicated in the following situations:

  • Allergy: If you are allergic to tirzepatide or any of the other ingredients (see PIL section 6).
  • Pregnancy: Mounjaro should not be used during pregnancy. If you are planning to become pregnant, stop Mounjaro at least one month before trying.
  • Breastfeeding: It is unknown if tirzepatide passes into breast milk – discuss with your doctor.
  • Severe gastrointestinal disease: Such as severe gastroparesis (delayed stomach emptying).
  • Personal or family history of medullary thyroid carcinoma (MTC) or in patients with Multiple Endocrine Neoplasia syndrome type 2 (MEN 2).

Warnings and precautions – discuss with your doctor if you have:

  • Pancreatitis (past or present)
  • Diabetic retinopathy (may worsen initially)
  • Severe kidney or liver problems
  • If you take sulfonylureas or insulin – your dose may need adjustment to avoid hypoglycaemia

🗒️ Note: Mounjaro contains benzyl alcohol (5.4 mg per dose). This can cause allergic reactions and, in large amounts, metabolic acidosis in people with liver or kidney disease. Tell your doctor if you have such conditions.

Should Mounjaro Be Taken With Food or Without Food?

Mounjaro can be taken at any time of day, regardless of meals. The injection is subcutaneous, so food does not affect its absorption. However, because Mounjaro slows gastric emptying, you may feel fuller sooner when you eat. This is part of its intended effect.

Some people experience nausea, especially after the first few doses. Taking the injection with a light meal or in the evening might help, but there is no requirement to take it with food. Always follow your doctor’s advice.

Special Conditions: Pregnancy, Breastfeeding and Other Considerations

Pregnancy

Mounjaro must not be used during pregnancy because the effects on an unborn child are unknown. If you are a woman of childbearing age, use effective contraception while taking Mounjaro. If you are planning a pregnancy, stop Mounjaro at least 4 weeks before trying to conceive. If you become pregnant while on Mounjaro, stop immediately and inform your doctor.

Breastfeeding

It is not known if tirzepatide passes into breast milk. A risk to the breastfed infant cannot be excluded. You and your doctor should decide whether to stop breastfeeding or to stop/avoid Mounjaro, considering the benefit of the medicine to you and the benefit of breastfeeding to your baby.

Driving and using machines

Mounjaro is unlikely to affect your ability to drive. However, if you take it with a sulfonylurea or insulin, hypoglycaemia could occur. Do not drive if you feel dizzy, shaky, or have other signs of low blood sugar.

Surgery

If you are due to have surgery under general anaesthetic, tell your doctor you are taking Mounjaro. They may advise you to stop it temporarily.

Children and adolescents

Mounjaro is not approved for use in children under 18 years.

Mounjaro Usage FAQs

Mounjaro may interact with other medicines, especially sulfonylureas or insulin (increased hypoglycaemia risk) and oral contraceptives (may be less effective for 4 weeks after starting or dose increase – use a barrier method). Always tell your doctor about all medicines you take.

If it’s been 4 days or less since your missed dose, take it as soon as you remember, then continue your usual weekly schedule. If it’s been more than 4 days, skip the missed dose and take your next dose on the scheduled day. Never take two doses within 3 days.

Appetite suppression begins within the first week, but maximum effects on blood sugar and weight take several weeks. Full steady state is reached after 4 weeks. Weight loss continues over months.

Occasional light alcohol is unlikely to cause problems, but alcohol can increase the risk of hypoglycaemia, especially if you also take sulfonylureas or insulin. It may also worsen nausea. Discuss with your doctor.

Store in a refrigerator (2°C – 8°C). Do not freeze. Once in use, you may keep the pen at room temperature (below 30°C) for up to 30 days. Always keep the pen cap on and protect from light.
